- ALL of the information above must be submitted by Midnight, Wednesday, December 28th to be considered
- Must be 35 or older
- Must be available for consultation, fittings and photo session several weeks prior to the event on January 23, 2015
- Be willing to sign a release of before and after photos
- Be open and willing to having hair cut and colored, makeup re-done and wardrobing suggestions
- Be willing to attend an exclusive event on January 7th for the announcement of winners, as well as the event on January 23, 2015
